In the event you do not have the ability to scan the five-page SERVICE REQUEST FORM, take pictures of all five pages and attach them to the ProtonMail® email you will send to us. Please ensure that the picture of each page of the SERVICE REQUEST FORM is in sharp focus and completely contained within each photograph. Also, when taking the pictures, position your phone camera directly above each page to ensure that the entire document can be read.
When emailing the SERVICE REQUEST FORM to us, include the word "CONFIDENTIAL" in the email subject line.
